This paper describes two incidents at steel making plant. These both occurred when liquid cast-iron was being drained from the bottom of the blasting furnace to empty the melting pot, into torpedo wagons. Spillage of the liquid cast-iron onto the ground occurred in both cases and generated an emission of airborne particles of reddish dust which was visible outside the plant.
Following the incidents, the plant operator carefully analysed the structural and mechanical causes of the accidents in order to minimise the chances of their re-occurrence and made technical modifications to optimise the transfer of the liquid cast-iron, reviewed operational and maintenance procedures and increased training for operators.
