In 2007 a batch manufacture of a fuel additive suffered a temperature excursion. The relief vent operated and the reactor burst releasing highly-flammable contents which exploded. Four people were killed and 32 injured, including 28 members of the public. The facility was destroyed, and buildings damaged up to ¼-mile from the site. This paper summarises the findings of the US Chemical Safety Board inspectors with additional comments by the author.
