Topic

There is a dilemma in process accident investigation statistics, in that, plants are becoming larger with more automation and reduced staff, yet increasingly accidents are being attributed to human error. This talk on process accident investigation is with examples of accidents in Malaysia and surrounding countries and their causes.

Speaker

aini_lingAini Ling has investigated over 900 fires and explosions in 10 countries and given evidence in court in three countries. Has undergone numerous advanced fire and explosion, thermal hazards and fire protection engineering training in USA and UK.

The industries that she inspects include oleochemicals, grains processors, foods, oil and gas, petro-chemical and power plants, in Malaysia, Thailand, Indonesia, Taiwan, Brunei, Hong Kong. Types of investigations include plant inspection, scrutinising process design, control and operational data as well as safety issues at incident plant.
