Overview

This presentation introduces a practical approach to human reliability analysis and its role in preventing major accidents. It explains how to identify safety-critical tasks and prioritise them according to their importance and the consequences of failure. The session explores how job design, individual capability and organisational conditions influence performance, and considers both the likelihood of error and opportunities to detect and recover from it before harm occurs.

Using worked examples of task assessment and barrier analysis, it shows how the findings can guide improvements to procedures, equipment labelling, communication and checking arrangements. Attendees will gain an understanding of how to focus attention on vulnerable tasks and strengthen the human contribution to process safety.

The presentation will be relevant to chemical engineers, process safety professionals and those responsible for operations, maintenance and competence manage.
