Overview
‘Disastrous Dinners’ are a series of informal events combining a short dinner with a presentation on past incidents, focusing on human factors and lessons learned. The series welcomes attendees from all disciplines, with case studies chosen to spark discussion beyond the process industries.
Piper Alpha thankfully so far remains the world's deadliest offshore disaster. This presentation sets out the asset and regulatory background before walking through the timeline of the events, the investigation's structure and findings, subsequent actions, and reviews the current success of their implementation. Aspects include Permit to Work (PtW) systems, simultaneous operations (SimOps) and Management of Change (MoC).
