Hold on to your safety hats as we buckle up for a rollercoaster ride of laughter (or agonising groans) as Andrew shares real-life anecdotes of nearly 15 years of HAZOP, LOPA and Functional Safety Encounters. From a risk matrix that seemed more like random chance than structured analysis to bizarre human decision making that will leave you wondering.

Amidst the mildly amusing stories, Andrew will shed light on the importance of robust methods for risk assessment and how functional safety places a crucial role in safeguarding processes.

Through this light-hearted presentation, you’ll gain valuable knowledge about HAZOP, LOPA and Functional Safety and maybe how not to phrase recommendations for chemists. Yet behind the stories lies profound messages about the importance of getting it right.

Speaker

Andrew Stead, Independent Process Safety Engineer

Andrew is an independent process safety engineer with a degree in chemical engineering. Andrew has worked in high hazard industries (including chemical / pharmaceutical / oil and gas) initially working across a number of roles in engineering (design, technical support, operations, and commissioning).

During this time he grew an interest in both process and functional safety, and began training and gaining experience in HAZOP / LOPA and other risk assessment techniques as well as becoming a certified Functional Safety Professional (TUV SUD).

As a veteran of well over 100 HAZOP/LOPA he runs his own company providing risk assessments and early FS lifecycle activities to local, national and international clients.
