We are pleased to announce that the latest issue of the Loss Prevention Bulletin (LPB) has now been published.

This issue of the Loss Prevention Bulletin brings together its usual diverse collection of case studies, technical analyses, and lessons learned in the process safety field.

It opens with an in‑depth retrospective on the 1944 Cleveland LNG disaster, revisiting its causes and the enduring relevance of its safety lessons 80 years on. The edition then explores the often‑overlooked risk of runaway chemical reactions outside traditional chemical sectors, illustrated through several recent incidents. A feature on “deferring to expertise” examines how organisations can strengthen process safety culture by empowering competent technical authorities.

Drawing on case studies outside the process industry, a review of the 2023 OceanGate Titan submersible implosion highlights failures in engineering assurance, organisational culture, and regulatory oversight, while human factors take centre stage in an investigation of the Haverfordwest paddleboarding disaster, analysing behavioural safety breakdowns that led to the tragic death of four participants.

The issue also includes an obituary for long time LPB contributor Douglas S Scott, a paper on a static‑induced ethyl acetate tanker fire, and a “near miss” case study involving a drum‑filling explosion.
