Loss Prevention Bulletin Issue 309

We are pleased to announce that the latest issue of the Loss Prevention Bulletin (LPB) has now been published.
Issue 309 brings together a diverse collection of case studies, technical insights, and lessons learned across the process safety community. This edition reflects on major industrial incidents, persistent engineering challenges, and evolving good practice.
The issue opens with a feature on the Alexander L. Kielland disaster, revisiting this significant offshore tragedy and the enduring lessons it offers for structural integrity and safety leadership. Further technical content includes Part 2 of our Monograph Series, focusing on vibration and small‑bore failures – a recurring cause of loss of containment.
Additional articles explore process safety learnings from ageing infrastructure and life‑extension work in brownfield environments, as well as insights from Simultaneous Operations (SIMOPS) events. We also mark the anniversary of the LG Polymers styrene release in Vizag, reflecting on the incident’s ongoing relevance for storage safety and emergency preparedness.
This issue concludes with new guidance from the IChemE Safety Centre on strengthening Management of Change, supporting organisations in improving control of change and reducing unintended risk.
