International Process Safety Day (IPSD) - Grenfell Tower Fire 2017
Gill Kernick, in conversation with Fiona Macleod, reflects on the role of leadership in learning from accidents. She shares her personal connection to the Grenfell Tower Fire and the subsequent public inquiry, challenges simplistic cause‑and‑effect explanations, and explores why our collective failure to learn is often predictable. She also introduces a framework for driving systemic change in complex systems.
We are continuing to promote this year’s International Process Safety Day (IPSD) and encourage anyone who could not join us live to explore the full program, which will remain accessible until the end of 2026. The complete agenda is available through the official program page, and all sessions can be viewed on the virtual platform. It is a great opportunity to revisit key insights, catch up on sessions you may have missed, and share the content with colleagues across the process safety community.
