Overview

This is the second of several web based training sessions on Risk Based Process Safety. In the first webinar the focus was on prevention, how to detect things going wrong and stop accidents happening. This second session will focus on preparing for the worst, if the preventative measures were not enough and we do have an incident, how do we minimise the harm to people and the environment - mitigation.

Mitigation: siting and layout; secondary and tertiary containment; relief systems; detection; abatement; fire fighting; blast resistant buildings; shelters; evacuation; emergency isolation; emergency response plans: internal and community; preparedness, communication teamwork and drills; incident investigation; incident analysis; organisational learning; leadership, culture and governance.

Speaker

Fiona Macleod, Professor in Process Safety, University of Sheffield

Fiona is a Chartered Engineer with over 40 years of hands-on industrial experience in the chemical and power industries. She graduated from Cambridge University, UK with a master’s degree in chemical engineering and went on to work in design, construction, commissioning, production, maintenance, and senior management all over the world for companies including Eastman Chemicals, Hovione, Victrex, ICI, Johnson Matthey and BP.

After 40 years in industry, Fiona switched to teaching process safety and loss prevention at Sheffield University, UK and joined the IChemE Safety Centre. Fiona also writes fiction under her married name of Fiona Erskine. 

Charles Cowley, Visiting Fellow in Safety Leadership, Cranfield University School of Management

Charles has been the CCPS staff consultant since 2012 and led projects writing books including:

  • Siting and Layout of Facilities’ 2nd Ed. (Wiley, 2018)
  • Bow Ties in Risk Management’ - CCPS jointly with the Energy Institute (Wiley, 2018)
  • Human Factors for Process Plant Operations’ - CCPS jointly with the Energy Institute (Wiley, 2022)

Charles represents CCPS at the European Commission Major Accidents Hazards Bureau. With 27 years at Shell, he worked in major projects, asset integrity, competence, leadership and culture. He led numerous safety audits and major accident investigations; led the ‘Hearts and Minds’ safety culture program in global downstream; and led team developing Shell’s operational risks control framework.

He developed and led the IChemE training course: Engineering Project Management, and is course director of IChemE training course: Process Safety Leadership and Culture. He is qualified as a commercial pilot and is a flight instructor.
