Overview
Disastrous Dinners is a series of informal events combining a short dinner with a presentation on past incidents, focusing on human factors and lessons learned.
The series welcomes attendees from all disciplines, with case studies chosen to spark discussion beyond the process industries.
Sixteen years ago, on August 17, 2009, an accident occurred at the Sayano-Shushenskaya hydroelectric power station, the largest in Russia. This event became one of the most significant man-made disasters in modern history. Who (or what) was to blame for the deaths of 75 people - equipment defects or staff negligence? What led to the “water hammer” and mechanical destruction of the turbine hall? The conclusions highlight the need for strict monitoring of equipment condition, improving safety culture and investing in the modernisation of aging power stations.
