Overview

Disastrous Dinners’ is a series of informal events combining a short dinner with a presentation on past incidents, focusing on human factors and lessons learned. The series welcomes attendees from all disciplines, with case studies chosen to spark discussion beyond the process industries.

This session focuses on the collision between the Solong and the Stena Immaculate in the North Sea in early 2025—a high-profile incident that made headlines nationally and was especially significant in the Humber region. The collision resulted in tragic losses and raised urgent questions around vessel coordination, communication failures, and decision-making under pressure. We’ll examine the events leading up to the incident, contributing human and systemic factors, and what the industry is learning in its aftermath.

Speaker

Joseph Hewitt, Project Engineer, Axi Design & Engineering & Director, Hewitt Associates Limited

Joseph is a Project Engineer educated in Chemical Engineering, with seven years of experience across the energy, construction, and marine infrastructure sectors. He has worked extensively on port fueling infrastructure for green hydrogen, methanol, and ammonia, as well as on shore power charging projects related to offshore wind operations and portside installations. Previously, Joseph contributed to innovation and development at Offshore Renewable Energy Catapult, and he has recently joined Axi Design and Engineering. Alongside his role at Axi, he is also a director of a family-run quantity surveying and project management consultancy. Joseph applies a practical, systems-based approach to delivering innovative and sustainable engineering solutions.
