Overview

This practitioner focused session with Alexandra Robertson explores why process safety risk across mining, processing and refining does not always arrive with a process safety label. Using major incidents and practical operating examples, the session examines how a credible loss of control scenario can become fragmented across production, maintenance, projects, critical controls, governance and incident classification.

Drawing on nearly three decades of experience across high risk operations, Alexandra considers how labels, organisational interfaces, degraded controls, normalised change and decision making can alter what an organisation sees, who owns the risk and whether warning signs trigger an adequate response.

Attendees will leave with a practical way to reconnect those fragments: start with the credible loss of control scenario, then test whether the organisation can still see it, own it, control it and learn from it.

Alexandra Robertson, Founder and Managing Director, 27South Advisory

Alexandra Robertson is an operations executive and chemical engineer with nearly three decades of experience across energy, mining, hydrometallurgical refining, infrastructure and bulk liquid storage and handling. She has held senior operational, engineering and process safety leadership roles, including Chief Operating Officer of Quantem Bulk Liquid Storage and Handling and General Manager of BHP’s Kwinana Nickel Refinery.

Her career has included responsibility for complex, high risk and asset intensive operations, major hazard facilities, multi site businesses and large engineering and operational teams. Her work has focused on strengthening operational governance, process safety, asset integrity and organisational capability, while improving the connection between technical risk, operational decisions and executive accountability.

Alexandra is a Fellow of IChemE and a Chartered Engineer. Through 27South Advisory, she provides independent advice on operational risk, governance, asset performance and transformation across high risk industries.
