On 2 June 2011, a team of contractors were preparing to enter an amine tank at the Chevron refinery in Wales. The tank exploded killing four people and permanently injuring a fifth. This talk will discuss what happened on the day and underlying process philosophy change which created the conditions resulted in the incident.

Disastrous Dinners is a series of talks focusing on an accident or disaster and what can be learnt from it. After the presentation will be a social dinner and drinks along with lots of time to continue the discussion.

Mark Bishopp, Principal Specialist Inspector, HSE

Mark Bishopp has worked for HSE for 36 years as a Specialist Inspector. He is currently a Principal Specialist Inspector, his responsibilties include taking the technical lead on flammable liquids, liaising with external bodies and in developing both internal and external guidance.

Time

18:30 arrival with a 19:00 start till 19:30. 

There is an optional social dinner after 19:30.
