Overview

Senior leaders play a critical role in ensuring that process safety risks are well managed within their businesses. Successive major accidents, including those at Piper Alpha, Texas City, Longford, and Buncefield have graphically demonstrated the catastrophic consequences that can occur, when senior leaders fail to set the right culture within their organisations, and are not sufficiently connected with activities at the front line.

With the HSE committed to programmes of process safety leadership inspections at Offshore and COMAH sites in the coming years, it is becoming increasingly important for individual operators to be able to practically demonstrate the role that their leadership plays, in the effective delivery of their major accident policies.

This presentation will share several examples of good practice that can help implement such policies, strengthen organisational culture, and ultimately improve overall process safety performance. These include examples in the areas of:

  • communicating key process safety messages and building a positive safety culture
  • delivering more effective site leadership visits
  • increasing workforce engagement and process safety competency
  • improving auditing and assurance.

Speaker

Ashley Hynds, Technical Director, WSP

Ashley is Technical Director for Process Safety with WSP, a global engineering services consultancy. He has helped many companies in areas such as safety cases, process safety leadership, permit to work and safe isolation and reinstatement of plant systems, competence, management of change, and auditing and assurance.
Prior to joining WSP as a Technical Director, Ashley has had over 25 years of experience in the chemicals and oil and gas sectors within ICI, INEOS and the HSE.

As Principal Inspector with HSE’s Energy Division – Offshore until 2021, Ashley held lead responsibility for process engineering, operations and process safety. As well as multiple offshore inspections, investigations, and safety case assessments, he contributed to many industry workgroups, and helped shape HSE’s policies. He was instrumental in initiating and shaping OGUK’s major HCR prevention plan, including its commitment to the Offshore Process Safety Leadership Principles, and helped define HSE’s Process Safety Leadership Intervention programme.

Earlier this year, he was joint winner of the IChemE Lees Medal for his paper on safe reinstatement of offshore process plant.
